I desire to enroll my daughter in the 2010 Fast Break Lacrosse Camp to be held between June 29 - July 2, 2010. I understand that Fast Break is not a function of the University of Virginia and that the University of Virginia is not responsible for camp activities. I understand that a risk of participating in any sport is the possibility of sustaining an injury. I hearby authorize the director and the professional training staff to act on my behalf according to her best judgment in any emergency requiring medical attention.

Applications will be accepted on a first-come, first-serve basis. Enrollment can only be secured by returning an application with a Deposit of $265.00 OR Payment In Full ($515.00). The deposit is non-refundable and will be deducted from the total fee. The balance is due by JUNE 1, 2010. The camp is owned and operated by Julie Myers, President of Cavalier Lacrosse, Inc.
